Some health conditions require you to provide a clearance letter from your treating physician documenting that your condition is stable on the current treatment plan and that you are medically cleared to safely operate a commercial vehicle.
Below is a partial list of some common conditions that require such a letter:
Diabetes Mellitus Type 2
Insulin-Treated - requires the ‘Insulin-Treated Diabetes Mellitus Form’ completed by the treating physician.
Non insulin-treated - a letter may be required including the most recent Hemoglobin A1c result and effectiveness of current treatment plan.
A blood glucose level may be checked during the examination.
Hypertension - poorly controlled high blood pressure (greater than 140/90) may require a reduced certification period or even disqualification.
Heart Disease - includes arrhythmias, coronary artery disease, heart attack (MI), congestive heart failure (CHF), valvular heart disease. A clearance letter is required from the treating cardiologist.
Sleep Apnea - a CPAP record is also needed
Psychological disorders - may require a letter from the appropriate health care profession-al documenting that the treatment has been shown to be adequate/effective, safe and stable (for example the use of sedating anti-anxiety medications such as benzodiazepines, sleeping medication, or stimulant treatment for Adult ADHD)
Chronic use of narcotic pain medications
Please inform the office if any of the above conditions apply to ensure that we have the necessary documentation before starting the examination.
